Shared Cost:

The Shared Cost Program prevents or delays inappropriate or premature institutional placement, allowing seniors 60 and older to remain at home for as long as possible.  Seniors share in the cost of services based on their income and monthly expenses increasing their accessibility to services.

The Shared Cost Program targets seniors who are unable to qualify for state funded programs due to long waiting lists or specific eligibility requirements.  To qualify eligible seniors must have two limitations in their activities of daily living and have no capable caregiver.

An initial assessment, case management, and ongoing homemaking, personal care, and emergency alert response systems are the primary program services.  Adult day care and in-home respite may also be included.  Seniors recuperating from an acute illness participate in the program only as long as services are required. This particular help can make the difference in getting well or returning to the hospital.

The seniors participating in the Shared Cost Program desperately want to remain at home. With a little help they can.

An in-home assessment and financial eligibility determination are required.
