In-home services questions, general information and eligibility rules.

You can call 321-806-3802 to inquire about services through our funded programs which offer in home personal care, light homemaking, respite care (in home or in facility), case management, companion, consumable medical supplies and emergency alert. You will schedule an over the phone assessment about your abilities which can take 30-45 minutes. This assessment with rate your needs compared with other’s in the community and will be placed on a list for services.

Another option is to call 321-639-6652 for Long Term Care managed Medicaid or Private Pay Home Care.

You can contact Senior Resource Alliance at 4074-514-1800 or 1-80-963-5337 to apply for Long Term Care Medicaid. If you are just trying to learn about how to apply or if you even qualify, you can contact SHINE at 1-788-416-5656.

We offer in home services to people 60 years and older who are living in their home. We offer personal care, light homemaking, respite (in home or in facility) case management, companion, emergency alert system and consumable medical supplies. Some services are private pay, state or federally funded or are billed to certain Long Term Care managed Medicaid. To learn more about which services you are eligible for through our Funded programs please call 321-806-3802. For our Private Pay or Long Term Care Managed Medicaid, please call 321-639-6652.

Some of our services are state and federally funded so there might only be a nominal co-pay, others are donation based, Private pay or through you Long Term Care Managed Medicaid.
